During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 4/1/2025, as a part of the licensure period. The on-site inspection began at 10:30 am and ended at 11:15 am. The inspector reviewed compliance in the areas listed above. There were (6) children present and (1) caregiver until the other caregiver arrived at 10:45 am; the point total was (17). The inspector reviewed (3) children?s records and (2) caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-460-D
The provider shall maintain documentation of current rabies vaccinations.
The rabies vaccinations for 2 household dogs was expired by 3 months.
Plan of Correction: The dogs will get their rabies vaccinations updated.
Standard 8VAC20-800-530-F
No soft bedding of any kind shall be used under or around infants including, but not limited to, pillows, quilts, comforters, sheepskins, or stuffed toys.
Two stuffed toys were found in the crib of child # 1, 11 months old who was observed sleeping.
Plan of Correction: The stuffed toys will be removed from the crib.
Standard 8VAC20-800-570-A
The provider shall ensure that a caregiver does not exceed 16 points.
From 9:30 am until 10:45, caregiver #1 exceeded 16 points. There were 6 children, totaling 17 points with caregiver #1 until 10:45 am when caregiver #2 arrived back to the house from an appointment.
Plan of Correction: The provider will be aware and maintain 16 points for each caregiver.
