During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/19/2026. The on-site inspection began at 9:55 am and ended at 10:50 am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 14. The inspector reviewed 5 children?s records and 4 caregiver/household member(s) records.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.
An updated TB screening for Caregiver #1 was not completed and was overdue by three days.
Plan of Correction: The caregiver will schedule an appointment for the TB screening.
Standard 8VAC20-800-350-E
REPEAT VIOLATION:
Hot water at taps available to children shall be maintained within a range of 105?F to 120?F.
The water tap in the kitchen measured 125.5 degrees.
Plan of Correction: The provider will notify the landlord that the water temperature needs to be lowered. The caregivers will carefully monitor the children during handwashing.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
The provider did not conduct a monthly evacuation drill for December 2025, January and February 2026.
Plan of Correction: Fire drills will be conducted and documented monthly.
Standard 8VAC20-821-170-B
Any person 14 years of age or older living in the home of a licensed or registered family day home shall request a Virginia child protective services central registry check within seven days of reaching 14 years of age or beginning to live in the home.
Household member #1 did not have central registry search within seven days of reaching 14 years and is overdue by 7 weeks.
Plan of Correction: The provider will complete the central registry search for the household member and email the results to the inspector.
