Inspector Notes

An announced initial inspection was initiated and completed on 7/19/24, as a result of a planned change in ownership. The classrooms to be used for children in the day care are classroom # 4, 5, 6,7 and 8 only.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/12/24. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center shall be maintained in a clean, safe and operable condition.
The floor air vents in the classrooms were easily removed by the hand or foot ,as a result there was a hole in the floor , which presented a head entrapment hazard.
Plan of Correction: The floor vent was secured with screws before the Inspector left. All the vents in the classrooms were checked and tightly secured to make sure it was securely placed.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.
1. Underneath and surrounding the play equipment on the primary playground, the mulch was compacted, had degraded to soil and no longer a resilient surface.
Plan of Correction: New mulch was ordered for the entire playground. Will be taking out 8 inches of the compacted mulch and replacing it with 8 inches of new mulch.

The applicant ( Person A) submitted a false liability insurance document as part of their initial application.
