During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and concluded on 10/22/24.The onsite inspection began at 12 noon and concluded at 2:40pm. The inspector reviewed compliance in the areas listed above. There were 46 children present and 6 staff. The inspector reviewed 10 children?s records and 9 staff records on-site.
This inspection included document review tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-550-P
The center shall maintain a written record of children's injuries in which the injures are documented the day of occurrence to include ways to prevent recurrence.

The injury/accident form being used does not have a place to document ways to prevent recurrence of injuries.
Plan of Correction: We will add this statement to all of the injury reports.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy from a physician to address the food which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction.
There was no written allergy care plan on file for children #2, #4, #5, #6, #7, #8 and #10, all had diagnosed food allergies.
Plan of Correction: We will obtain the allergy care plans for those children with food allergies.
Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance.

For child #1 through #10, there was no written agreement on file between the parents and the center to address seeking emergency medical are, picking up the child when they become ill and reporting cases of communicable disease in the child's home to the center. The 10 children enrolled two months ago.
Plan of Correction: We will audit all children's files and obtain a written agreement from the parents.
