During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/25/2025. The on-site inspection began at 12:00pm and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and three staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #4, who had been attending for six months, did not have documentation of an immunization record on file.
Plan of Correction: The center will obtain the immunization records for the child.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4, who had been enrolled for six months.
Plan of Correction: The center will obtain the physical for the child.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The hand railing leading from the outside deck to the lawn was loose needed repair in order to prevent children from falling down the stairs.
Plan of Correction: The center will have the railing repaired.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #4, enrolled for six months, was missing the name, address, and phone number of a second designated person to call in an emergency if a parent cannot be reached.
Plan of Correction: The center will obtain information for a second emergency contact.
