During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 3/25/2026 and completed on 4/6/2026 with a local agency in response to a complaint received by the licensing office on 3/25/2026 relating to allegations of age appropriate activities and reporting of a child's injury. The inspector reviewed one children?s records and two staff records on-site. This inspection included document review, tour of the facility and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/13/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 3/18/2026 Staff #1 set up and supervised an outdoor activity that included lighting sticks and paper on fire in a tin foil bowl for the children to jump over while participating in a cultural celebration. After the activity, Child #1 picked up a hot stick, causing an injury, that resulted in a pea sized blister on his hand.

The variety of daily activities for all age groups shall be age and stage appropriate.

Staff #1's activity, using live fire, was conducted with five year old children on the playground. A bowl of sticks and scraps of paper were set on fire for the children to jump over.
Plan of Correction: Any activities involving fire are not permitted

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

On 3/18/2026 Child #1 sustained a minor burn injury on his hand. An injury report was not completed until 3/24/2026.
Plan of Correction: Injury report was made on 3/24/2026. All staff reminded to make report day of accident
