During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 11/22/24 and completed on 11/22/24. The on-site inspection began at 10:30am and ended at 12:55pm. The inspector reviewed compliance in the areas listed above. There were 82 children present and 17 staff. The inspector reviewed 8 children's records and 8 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff 1 began employment prior to the results of the national criminal background check being received.
Plan of Correction: The center responded with the following: Going forward, center management will ensure that staff do not begin employment until after their fingerprint results have been received.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

The screening for staff 1 was completed four days after hire.
Plan of Correction: The center responded with the following: Going forward, center management will ensure TB screenings are completed within the required timeframes.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. Resilient surfacing of 6 inches is required as per the American Society for Testing and Materials.

The mulch surrounding the climbing structures on the playground measured one inch in depth.
Plan of Correction: The center responded with the following: New much will be ordered as soon as possible. Center management will monitor the depth of the mulch on the playgrounds on a routine basis and order new mulch as needed.

Staff records shall include documentation to demonstrate that the individual has completed orientation training.

The record for staff 2, who has been working at the center for one month, does not contain documentation that orientation training has been completed.
Plan of Correction: The center responded with the following: Staff 2 received orientation training upon employment; however, it was not documented in the staff record. Documentation of orientation will be completed and placed in staff 2's record.
