Inspector Notes

An unannounced, on-site monitoring inspection was completed on 7/31/2024, as a part of the conditional licensure period. The on-site inspection began at 10:10am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 76 children present and 11 staff. The inspector reviewed 2 children?s records and 1 staff record. This inspection included: document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from 9/18/24, which will be the close of business on 9/25/24. A POC submitted after this date will not appear on the public website.

Based on review, the facility did not ensure that staff completed orientation training prior to working alone with children an no later than seven days of the date of assuming job responsibilities.
Evidence: reviewed 1 staff record and found that that staff member did not have documentation of completing orientation training, and had been employed for 6 weeks.

Standard 8VAC20-780-460-A
Based on observation, the facility did not ensure that at no time there are fewer than two staff members supervising swimming activities, and that designated certified lifeguards are not counted in the staff to child ratios.
Evidence: observed that the certified lifeguard was actively supervising 3 children by themselves, and teaching the children to swim in the facility pool.
