Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 4/15/25 from 10:45 am to 11:40 am. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 8. The inspector reviewed 2 children?s records and 1 caregiver's record on-site. This inspection included document review, tour of the family day home,
interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider must obtain documentation of a physical exam for children within 30 days of enrollment.
Child # 1 has been in attendance for 7 months. The provider did not obtain a copy of the physical exam for Child # 1 to this date.
Plan of Correction: Copy of the physical exam will be requested from the parents for the file.
Standard 8VAC20-800-50-A
The family day home shall keep a written record of children in attendance each day.
The attendance log did not have any recorded attendance for the month of April 2025. Additionally, attendance for March 2025 was not accurately documented.
Plan of Correction: Attendance will be recorded accurately and daily.
Standard 8VAC20-800-60-B
Children's records must have complete information.
Child # 1 has been in attendance for 7 months. Child # 1's record had incomplete forms for the following: medication administration policy, liability insurance policy, doctor's information was missing, medical insurance policy information, authorization for emergency medical care was not signed by the parent.
Plan of Correction: All incomplete forms will be completed and signed.

The provider must obtain documentation of immunizations for children before the first day of attendance.
Child # 1 has been in attendance for 7 months. The provider did not obtain immunization records for Child # 1 to this date.
Plan of Correction: Immunization records will be requested from the parents for the record.
