Standard 8VAC20-800-280
Cleaning agents shall be inaccessible to children.

Cleaning agents were found in a kitchen cabinet that was accessible to a children. A child was observed in the kitchen area.
Plan of Correction: The provider locked the cabinet during the inspection.
Standard 8VAC20-800-290
Sharp kitchen utensils and other sharp objects shall be inaccessible to children.

Knives were located in a kitchen drawer accessible to children. A child was observed in the kitchen area.
Plan of Correction: Safety locks have been put on all lower cabinets.
Standard 8VAC20-800-430-C
Children over the age of two shall not have access to stairs that do not have protective barriers.

The staircase leading to the second floor did not have a protective barrier. A child, over three years old, was observed moving freely throughout the first floor. While the child occasionally walked alongside the provider, they were also seen running away from the provider at times.
Plan of Correction: Protective barrier gate has been put up in the hallway that leads to the staircase; now inaccessible to children.

The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.

The annual emergency plan review was overdue by six and a half months.
Plan of Correction: Annual schedule emergency plan reviews are now implemented. Last review was done after inspection on 10/30/2025
