Inspector Notes

An announced initial inspection was completed on 05/02/2024 as a result of a planned change of ownership. The physical space, one staff record, the injury prevention plan, playground safety procedures, menu and Emergency Preparedness Plan supplies and required postings were viewed.

Any areas of noncompliance are documented in the violation notice.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be corrected. Please do not use caregiver names; list caregivers by position.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and the public website.

Based on observation, areas and equipment of the center were not maintained in a safe and operable condition.
Evidence:

1.The resilient surfacing tiles on the young toddler and early preschool playgrounds are warped and pose a tripping hazard.
2. There are exposed wires on the net climber on the main playground.
3. There is warped wood that is splintered around the edges of the sand box.
