Inspector Notes

An unannounced investigation was initiated on 07/08/2024 and concluded on 07/11/2024 regarding a self reported incident. Interviews were conducted and program policies and training were reviewed. After interviewing involved parties, violations were issued and can be viewed on the Violation Notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). If a plan of correction is not received within the required timeframe, it will not appear on the public website.

Standard 8VAC20-780-340-A
Based on interview, when supervising children, staff did not always ensure care, protection and guidance.
Evidence:
1. Staff #1 confirmed that on 07/05/2024, Child #1 (age 18 months) was served yogurt at snack time, exposing the child to an allergen.
Staff #1 did not verify Child #1's complete documented food allergen list before serving Child #1 yogurt.
2. Child #1 suffered an allergic reaction and had to be administered Zyrtec and epinephrine injection then 911 was called and child was transported to the hospital.
Plan of Correction: In order to keep the students safe moving forward, we terminated Staff #1 that day.

We edited the July Menu to include the Late PM Snack to ensure families and staff know what is being served from drop off to pick up
?
I added new allergy profile sheets for every student with epi-pen allergies that are now posted on the outside of the cabinets, covered in a red sheet to ensure that teachers know that there is a student with allergies in that room

Every student with allergies will be served their lunched on a red plate to ensure they are given the correct meal/food
?
I did an Allergy Re-Training with every staff members to ensure they understand the importance of our Master Allergy List and that we always check it prior to serving food
?
I discussed in the training what students have personalized menus and they are to be followed
?
In order to personalize every classrooms, I made an Allergy List & Preference list for each hallway that includes pictures of those with allergies and preferences

Every staff member received an Allergy Care Quick Guide with the written out policies and procedures

Based on interview, a child diagnosed with a food allergy was served a food identified on their written allergy care plan.

Evidence :
Staff #1 stated that " I quickly gave , Child #1 (age 18 months) the yogurt in between serving everyone else's snack". Child #1 has a diagnosed food allergy and the center was provided with an allergy action plan. Child #1 had a serve allergic reaction to the yogurt that was served.
