During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 3/25/2025, as a part of the enforcement process. The Department issued a Notice of Intent to require the licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 1/3/2025 as a result of high-risk violations. The on-site inspection in response to a self -report received by the licensing office on 3/21/2025 relating to Supervision of Children began at 10:30 am and was completed at 12:40 pm. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.


Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/23/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure care, protection and guidance.

1.Staff #1 confirmed that on 03/20/2025, Child #1 (age 26 months ) was served a known allergen at lunch time, exposing the child to an allergen. Staff #1 ( in training) did not verify Child #1?s identity before serving child # 1
2. Child #1 showed symptoms of an allergic reaction and had to be administered an over the counter antihistamine and an emergency medication ( per parents request). The center called 911 and the child was transported to the hospital.
Plan of Correction: As of 3/21 our plan of correction is to require name-to-face check with those who have food allergies/preferences when employees are entering the room for coverage during snack and mealtimes.

As of 3/21 our plan of correction is to require a separate Allergy Orientation Training alongside the required trainings from Creme de la Creme.

A child diagnosed with a food allergy was served a food identified on their written allergy plan.

Staff #1 stated that ?I was asked by Staff #2 to help pass out drinks. I poured it and passed it out to everyone at the table.? Child #1 ( age 26 months ) was served the drink even though the child has an allergy care plan with a diagnosed food allergy to the drink. Child #1 showed symptoms of an potential allergic reaction and was given medication by the center.
Plan of Correction: As of 3/21 our plan of correction is to require all new employees to not serve any snacks, lunches, or drinks until they have been employed for 30 days.
