During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced on-site monitoring inspection was initiated and completed on 6/3/2025, as a part of the enforcement process. The Department issued a Notice of Intent to require the licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 1/3/2025 as a result of high-risk violations. The on-site monitoring inspection began at 10:00 am and concluded at 12:15 pm. There were 71 children directly supervised by 18 staff. The inspector reviewed 4 child records, 5 staff records, medication documentation , a tour of the facility and interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on the violation notice issued to the program.
Violations
32 bikes on the playground had broken and cracked plastic connecting the handlebars to the bike. The cracked plastic was sharp to the touch.
There were unlocked cleaning materials on white shelf and in the bathroom.
The record for child A did not contain parental permission for child to be released to school bus daily and return into care.
