During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on 6/3/2025, as a part of the enforcement process. The Department issued a Notice of Intent to require the licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 1/3/2025 as a result of high-risk violations. The on-site monitoring inspection began at 10:00 am and concluded at 12:15 pm. There were 71 children directly supervised by 18 staff. The inspector reviewed 4 child records, 5 staff records, medication documentation , a tour of the facility and interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include visible cracks.
2 bikes on the playground had broken and cracked plastic connecting the handlebars to the bike. The cracked plastic was sharp to the touch.
Plan of Correction: Bikes were removed during the inspection.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
There were unlocked cleaning materials on white shelf and in the bathroom.
Plan of Correction: We locked during the inspection.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain any blanket permission slips.
The record for child A did not contain parental permission for child to be released to school bus daily and return into care.
Plan of Correction: We emailed mom during the inspection to get written permission.
