During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on 10/9/2025 and is a part of the enforcement process. The Department issued a Notice of Intent to require the licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 1/3/2025 as a result of high-risk violations. The on-site monitoring inspection began at 11:00 am and concluded at 1:30 pm. There were 86 children directly supervised by 20 staff. The inspector reviewed 6 child records, 5 staff records, medication documentation, a tour of the facility and interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on the violation notice issued to the program.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

There is peeling paint on shelves and toy shelf in two rooms.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain required information.

The record for child #1 is missing parent work addresses.
