Inspector Notes
An unannounced, on-site monitoring inspection was initiated on and completed on 3/16/2026, as a part of the licensure period. The on-site inspection began at 7:50 am and ended at 9:25 am. The inspector reviewed compliance in the areas listed above. There were 38 children present and 11 staff. The inspector reviewed 4 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Staff A who has been employed for approximately one month, did not have documentation of request for a central registry check and a sex offender search for a different state.
