During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on October 23, 2024. The on-site inspection began at 10:00am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 27. The inspector reviewed 7 children's records and 2 caregivers.

This inspection, included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-320-I
The absence of smoke detectors as required by the Uniform Statewide Building Code and the Statewide Fire Prevention Code. The provider shall comply with the requirements made by the fire prevention or building officals to eliminate fire hazards.

The smoke detector was disassembled and missing batteries.
Plan of Correction: I had taken it down to replace the batteries. I have installed the new batteries and it is now fixed. I will be sure to replace the batteries immediately.

Standard 8VAC20-800-330-A
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation.

There were only two (2) cordless telephones available during the inspection.
Plan of Correction: I will buy a phone that is not cordless tomorrow.
Standard 8VAC20-800-680-B
Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after diapering.

Child #3 hands were not washed after diapering.
Plan of Correction: Handwashing procedures were reviewed with the caregiver and will ensure hands are always washed after diapering.

The diapering surface shall be cleaned and sanitized after each use.

The caregiver did not clean and sanitized the diapering surface after each use.
Plan of Correction: The cleaning procedures were demonstrated to the caregiver. The surface will be cleaned and disinfected after each diaper change.
Standard 8VAC20-800-750-A
When topical skin products such as diaper ointment are used, the product shall be labeled with the child's name.

Three tubes of diaper creams were not labeled with a child's name.
Plan of Correction: I will write the names on the cream now. I will be sure the names are always on the creams.

Topical skin product, such as diaper cream, shall not be used beyond the expiration date of the product.

Destin diaper cream had an expiration date of 06/2024.
Plan of Correction: I did not notice the cream had expired. I will be sure to check in the future.

A first aid kit shall contain at a minimum of triangular bandages.

The kit did not have more than one (1) triangular bandage available.
Plan of Correction: I will buy another triangular bandage today.
Standard 8VAC20-800-90-A
Before attendance, documentation of immunization is to be obtained for each child.

Documentation of immunization for Child #1 was obtained over 4 weeks after attendance. Documentation of immunization for Child #3 was obtained 3 weeks after attendance. Documentation of immunization for Child #7 was obtained 2 days after attendance.
Plan of Correction: The provider will obtain the immunization record on or before the child attends.
