During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on May 5, 2025. The on-site inspection began at 10:10am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were seven children present and two caregivers; the point total was 25. The inspector reviewed seven children?s records and three caregiver/household members records on-site. This inspection included document review, tour of the home, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 8VAC20-800-280
Potentially poisonous materials shall be stored in an area inaccessible to children. Four bottles of hair products and one bottle of shampoo for dogs were on the bathtub ledge in the central bathroom that is used for the children. The label had "Keep out of reach of Children".
Plan of Correction: I will keep them in the cabinet that is locked. Violation corrected during the inspection.
Standard 8VAC20-800-680-A
Repeat Violation: Caregivers shall wash their hands with liquid soap and warm running water after diapering a child or assisting a child with toileting. Caregiver #2 did not wash her hands diapering a child.
Plan of Correction: I will go over the diapering and handwashing procedures to ensure we are following the guidelines. We will wash our hands after diapering.

Repeat Violation: Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after toileting or diapering. Caregiver #2 did not wash a child's hands after diapering.
Plan of Correction: Diapering and hand washing procedures were reviewed . We will wash all children's hands after diapering.
Standard 8VAC20-800-690-E
Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. The foot operated storage system was turned facing the wall, and caregivers #1 and #2 used their hands to open the lid to dispose of soiled diapers.
Plan of Correction: I had the storage container turned because I did not want the children to play or step on the pedal. I will turn it around now and we will use the foot-pedal to dispose of the diapers.

Standard 8VAC20-800-90-C
Documentation of additional immunization is required every six months for children under the age of two years. Child #1, 22 months old, last documentation was nine months ago, Child #3, over 16 months old, last documentation was ten months ago, and Child #4, 15 months old, last documentation was eight months ago.
Plan of Correction: I will tell the parents today to bring in the updated immunization by May 9th.
Standard 8VAC20-800-960-E
Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care. There were two bottles of milk inside a plastic bag not labeled with the child's full name and date. The violation was corrected during the inspection.
Plan of Correction: I will put the child's name and date on the label now. We will label all food brought from home. Violation corrected during the inspection.
