During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced on-site renewal inspection was initiated and completed on September 18, 2025. The on-site inspection began at 11:05am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were six children present and three caregivers; the point total was 21. The inspector reviewed six children?s records and four caregiver/household members records on-site. This inspection included document review, tour of the home, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4The record for Child #1 was missing the address for both emergency contact persons. The child was present during the inspection, and had been in care for eight months.
Documentation of additional immunizations is required once every six months for children under the age of two years. 'The provider did not receive documentation once every six months for Child #4, who is currently 21 months. The documentation was received eleven months after the initial immunization record was provided to the provider.
