Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 7/25/2024 as a part of the conditional licensure period. The on-site inspection began at approximately 12:48 pm and ended at 3:00 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 7 staff. The inspector reviewed 5 children?s records and 12 staff records. This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/2/2024. A POC submitted after this date will not appear on the public website.
Violations
3Evidence:
1. The record of staff #4 (date of employment: 4/11/2024) did not contain documentation of completion of the required course.
2. Administration acknowledged that staff #4 did not complete the course within 90 days.
Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. The exposed sharp ends of four nails were observed protruding from the wooden playground fence and three rusted bikes were observed on the playground.
2. Administration acknowledged that the fence and bikes required maintenance to be safe.
Evidence:
1. The records of child #3 (date of enrollment: 6/3/2024) and child #5 (date of enrollment 6/3/2024) were missing one emergency contact address. Child records are required to contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
2. Administration acknowledged the records were incomplete.
