During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/31/2025 and completed on 4/1/2025. The on-site inspection began at 9:28 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 32 children present and 11 staff. The inspector reviewed 5 children?s records and 4 staff records on-site and 1 staff electronically on 4/1/2025. This inspection included document review, a tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/8/2025. A POC submitted after this date will not appear on the public website.

Centers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #3's fingerprint background check was received 1 day after the start of employment.
Plan of Correction: This staff was hired in 3/24. The new HR director does not let staff start prior to a completed background check.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #2 had a TB screening that was completed prior to employment, approximately 12 days outside of the 30 day window.
Plan of Correction: All TB tests will be done within the 30 days before hire.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff #3 completed the department sponsored orientation over 9 months late.
Plan of Correction: All staff will complete preservice in 1st 90 days.

Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Child #1 had two prescription diaper creams that were not labeled with the child's name.
Plan of Correction: All diaper creams are labeled.
Standard 8VAC20-780-510-I
The center may administer prescription medication provided: 1. The medication is administered by a staff member or an independent contractor who meets the requirements in 8VAC20-780-245 J; 2. The center has obtained written authorization from a parent or guardian.

There was no written parent/guardian or physician authorization to administer prescription medications to child #1. Staff stated that two prescription diaper dreams had been administered daily to child #1 for approximately 1 week. The staff stated that they did not have the certification/training required to administer prescription medications.
Plan of Correction: All medication will be administered by a MAT trained staff member. The proper authorization forms will be filled out.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: 1. Date and time of injury; 2. Name of injured child; 3. Type and circumstance of the injury; 4. Staff present and treatment; 5. Date and time when parents were notified; 6. Any future action to prevent recurrence of the injury; 7. Staff and parent signatures or two staff signatures; and 8. Documentation on how parent was notified.

An injury report for child #6 was missing one of the two required signatures.
Plan of Correction: Injury reports will be reviewed for completion including two signatures.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

Child #2, enrolled 1 week, and Child #4, enrolled over 3 weeks, were missing emergency contact addresses. Child records are required to contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: The required addresses have been obtained. No child will start with incomplete forms.

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #5, who has been enrolled for over 3 weeks, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: The child was provided with an appropriate substitution and the action plan will be obtained.
