During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 8/06/2025 and completed on 8/15/2025 in response to a self-report received by the licensing office on 7/28/2025 relating to supervision and behavioral guidance. The inspector reviewed 1 child?s record and 6 staff records on site. This inspection included document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background checks and first aid and CPR training and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/22/2025. A POC submitted after this date will not appear on the public website.
Violations
6Staff #1, employed for over one week, worked alone with children on 7/25/2025 for approximately one hour. At the time, the center did not have the results of Staff #1's out of state child abuse and neglect background check. Staff #6, employed for over one month, had an out of state child abuse and neglect background check that was requested 13 days after the start of employment.
On 7/25/2025, Staff #1 placed Child #1 in the bathroom during a nap period that lasted approximately two hours. Staff #2, who was responsible for supervising the second hour of naptime, observed that Child #1 and their mat were not in their previously designated rest space. Staff #2 stated that they were not aware that Child #1 was in the bathroom and thought the child had been picked up. Staff #2 did not attempt to verify Child #1's attendance with administration.
On 7/25/2025, Child #1 (age 3) was placed in the classroom bathroom by Staff #1 for approximately two hours during naptime. Staff #1 turned off the bathroom light and closed the bottom half door which left Child #1 out of the staff member's sight. Staff #2, who supervised the classroom for approximately half of the two hour naptime, stated that they were unaware that Child #1 was in the bathroom after returning from their break.
On 7/25/2025, the center did not follow their procedures for identifying where children are at all times. The center's policies and procedures require that "Teachers need to be aware of the number of children in their care at all times. Teachers must do head counts of the children in their care frequently. If two or more teachers are working together, they need to communicate regularly regarding the number of children in their care. Children must be supervised at all times including rest times and while using the restroom." Staff #2 stated that they were not aware that Child #1 was in the bathroom during the one hour period where they supervised the classroom. Staff #2 also stated that they did not communicate about the supervision of the children before assuming responsibility of the classroom from Staff #1. The center's administration acknowledged that Staff #2 did not follow the center's procedures.
On 7/25/2025, Staff #1 placed Child #1 (age 3) in the bathroom during nap. Staff #1 stated that Child #1 was being disruptive and was moved so the other children could sleep. Staff #1 stated that they were frustrated and acknowledged that their actions were not constructive or appropriate.
On 7/25/2025, Staff #1 was alone for approximately one hour with a group of ten children (ages 22 months to 3 years) and was not certified in CPR and first aid.
