During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was conducted on 3/18/2026 in response to a complaint received by the licensing office on 3/10/2026 relating to allegations in the areas of supervision, handwashing procedures, and the prevention of the spread of disease. The inspector reviewed 1 child record and 1 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation supports part of the allegation, therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/26/2026. A POC submitted after this date will not appear on the public website.
Violations
1If a child needs to be excluded according to subsection A of this section, the following shall apply: 1. Arrangements shall be made for the child to leave the center as soon as possible after the signs or symptoms are noticed; and 2. The child shall remain in the designated quiet area until leaving the center.
On or around March 3, 2026, Child #1, age 2, recurrently vomited in the classroom. Child #1 was kept in the classroom with 8 additional children for approximately one hour before being brought to the center's designated quiet area to await pickup.
