During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 6/26/2025 and completed on 6/26/2025 in response to a complaint received by the licensing office on 6/20/2025 relating to allegations of supervision and following policies and procedures. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/25/2025. A POC submitted after this date will not appear on the public website.

The required ratio for children ages 16 months to 2 years old is one staff per five children.

On 3/28/2025, Staff #1and Staff #2 were found to be supervising 14 children (youngest child 22 months) on the playground from approximately 10:30am to 11:02am.
Plan of Correction: Ensure children return from playground with correct class grouping.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

In Section 5 page 1 of the center's Policies and Procedures about Safety in the Workplace states that "Staff members will conduct a head count at least once an hour and when children return from playground. Head count totals will be checked against Brightwheel class list. Head count must match number of children listed in attendance on Brightwheel." Staff completed a head count that did not match the true number of children in their class or attendance through Brightwheel.
Plan of Correction: Reviewed name to face headcount and stressed that late arrivals should be double-checked with staff to prevent future issues.
