Inspector Notes

An announced initial/change of ownership inspection was conducted on 5/22/2024 from 10:14 am-12:30 pm. There were 16 children supervised by 4 direct care staff. The physical plant, 5 staff records, 5 children?s records, staff qualifications, background checks, policies and procedures and the emergency preparedness plan were reviewed. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Rooms are filled with supplies, equipment, and age appropriate activities for children. Areas of non-compliance are identified in the Violation Notice. Based on record review and interviews, the applicant did not ensure to a obtain a copy of the results of a search of the central registry (CPS) for agents/board officers.

Evidence: There was no documentation of virginia central registry results for board officer #2 who is listed as a member on the business entity page.
Plan of Correction: Application for CPS search was submitted and fees paid to OBI on 5/22/2024. Results will be provide when received from OBI.
