Inspector Notes

An unannounced, on-site renewal inspection was initiated on 11/12/2024 and completed on 11/12/2024, as a part of the conditional licensure period. The on-site inspection began at 10:00 am and ended at 12:10 pm. The inspector reviewed compliance in the areas listed above. There were 19 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/20/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas inside of the center were not maintained in a safe condition.

There was peeling paint found in a children's bathroom and in 2 out of 3 classrooms inspected. Peeling paint was found next to the children's toilet, sink, diaper-changing table, and cubby.
In 1 out of 3 classrooms inspected, there were two pieces of brown children-sized chairs with several rips and tears on the arms of the chairs.
Plan of Correction: The peeling paint has been repaired in all affected areas and have been repainted.

The damaged chairs have been repaired with new fabric replacements.

Inside areas occupied by children shall be maintained no lower than 68 degrees F.

The temperature in an occupied classroom was 65 degrees.
Plan of Correction: The temperature has been adjusted to 72 degrees F.
Standard 8VAC20-780-500-B
Disposable diapers shall be used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

During the inspection, the trash cans used for diaper disposal in 2 of the 3 classrooms were not hands-free. In one of the classrooms, the trash can lid was not properly connected to the trash can, as a result, it did not open automatically when the foot pedal was pressed.

In the second classroom, the trash can lid could be opened with a foot pedal, but there was a closed barrier within the trash can where a staff member would have to either use their hand or soiled diaper to push the barrier open to throw the diaper away.
Plan of Correction: The trash cans in all classrooms were fixed the same day.

In the second classroom, the trash can was replaced with a completely hands-free model.
