Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on 04/16/2025 and completed on 04/16/2025. The inspection started at 1:03pm and concluded at 1:53pm. There was one child present and one caregiver; the point total was 2. The inspector reviewed two children?s records and three caregiver/household members records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There was no attendance records maintained for over eleven months.

The vendor shall obtain documentation of additional immunizations once every six months for children less than the age of two years.

Child #2, age 2 years and who has been enrolled for one year, did not have documentation of additional immunization. The last documentation for immunizations was dated April 2024.
