Inspector Notes

An unannounced, on-site initial inspection was initiated on 5/14/2024 and the inspection concluded on 5/14/2024. The on-site inspection began at 10:00 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. The inspector reviewed three children's records and one staff record on-site. This inspection included document review, tour of the home, interview and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on record review, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
There was not documentation of a physical examination in any of the child records.
Plan of Correction: The provider will request physical examinations from parents.
Standard 8VAC20-800-350-E
Based on testing the temperature of hot water taps available to children, the provider failed to maintain a water temperature with a range of 105 degree F to 120 degree F.

Evidence:
Temperature of the water tap in the bathroom used by children was 132 degrees F.
Plan of Correction: The provider will have temperature reduce to within 105 degrees F and 120 degrees F.
Standard 8VAC20-800-460-C
Based on provider interview, dogs or cats, where allowed, shall be vaccinated for rabies and shall be treated for fleas, ticks, or worms as needed.

Evidence:
Provider reported that four dogs on the premises were not vaccinated.
Plan of Correction: The provider will ensure all pets on premises have required vaccinations.
