During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on June 12, 2025, and completed on June 12, 2025, as a part of the licensure period. The on-site inspection began at 10:00 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 20 children present and seven staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The most recent fire inspection was conducted fifteen months ago, exceeding the annual requirement.
Plan of Correction: Per the Director: I will contact our administrative office and see if they have an updated fire inspection. If not we will have one conducted.
Standard 8VAC20-780-510-P
Repeat
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. 1. The medication authorization for child #1 expired over two months ago. The medication was still on site. 2. The medication authorization for child #6 expired over two months ago. The medication was still on site. 3. The medication authorization for child #7 expired over seven months ago. The medication was still on site.
Plan of Correction: Per the Director: I will return all the medications with expired authorizations. Parents will be asked to complete new authorization forms and to get doctors authorization for long term medication.
