Plan of Correction: Per the director: I will make sure immunizations are obtained prior to enrollment.

Program leaders shall have fulfilled a high school program completion or the equivalent and have met one of the following experience and education requirements.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. In the two-year-old classroom a cleaning material was stored in the bottom cabinet unlocked.
Plan of Correction: Per the director: I will discuss with staff that all cleaning materials have to be locked. The cleaning material was removed and placed in a locked cabinet.

Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Prescription medication for child #2 did not have the original container or label. The medication was not labeled with the child?s name, the amount of dosage, and the time or times to be given.
Plan of Correction: Per the director: I will ask the parent to provide the label for the medication.

Medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Medication for child #2 and child #5 were observed unlocked in a cabinet above the sink.
