Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on 3/16/2026 and completed on the same date. The inspection started at 9:00 and ended at 9:45 . There were 3 children present and the provider; the point total was 7. The inspector reviewed 3 children?s records and 2 household members records during the inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The child's information

One child, child 3, is missing the emergency contact and doctors information as well as thge agreement between parent and provider.
Plan of Correction: I will ask for the information tonight at pick up.
Standard 8VAC20-790-430-B
The vendor shall maintain a record of the dates of the practice drills for one year. For vendors offering multiple shifts, the simulated drills shall be divided evenly among the various shifts.

The provider has drills recorded until September 2025, the paper with the drill from Aug 2025 to February 2026 can not be located at the time of the inspection.
Plan of Correction: I had it last week but i think someone in my household might have accidentally tossed it, if I find it I will send it to you.
