Inspector Notes
An unannounced inspection was initiated on December 13, 2024 and concluded on April 7, 2025 in response to a complaint received by the licensing office on November 20, 2025 relating to allegations of care and protection. The inspector reviewed 1 child?s record and 3 staff records on-site.
This inspection included document review, a tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 18, 2025. A POC submitted after this date will not appear on the public website.
Violations
1guidance. On November 19, 2024, the center was notified of a visible injury on Child 1 (age 5 months) that extended from the front to the back of the child's neck. The child also had a rash on the face. The parent took the child for medical care.
According to the teacher interviews and corroborating electronic reports, the child was pick up by her father on November 18. The father noticed a red line on the child?s neck and asked the teacher who said she saw the line and that the child had had a bib on at different times throughout the day, for feeding and drooling purposes, and only when the child was awake and alert (never in the crib or while sleeping) and therefore the red mark did not seem alarming to her. The father left the school, passing two members of management and the child?s lead teacher and did not mention any concerns. Several hours later, the mother arrived home and was concerned with a rash on the child?s cheeks so took the child to the doctor.
? Review of Classroom Materials:
All bibs were immediately removed from the classroom and evaluated. Although no materials were found to be defective, the school opted to proactively replace all bibs with new, high-quality bibs that are constructed from soft, breathable organic cotton and free of any materials that may cause friction or irritation (ex: Velcro).
? Staff Re-Training and Policy Acknowledgment:
Staff working with infants and young toddlers were re-trained on:
o Proper use and fit of bibs
o Daily visual wellness checks performed at drop-off
o Immediate communication and documentation procedures when physical changes, such as skin sensitivity or marks, are observed
o Staff signed updated acknowledgments verifying their understanding and compliance with the expectations and their responsibility in supporting child wellness and parent communication.
? Incident Reporting Policy Review and Reinforcement:
The school reviewed and reinforced its internal incident reporting procedures to ensure that any observable changes in a child?s appearance or condition are promptly communicated and appropriately documented.
