Inspector Notes

An unannounced, on-site non mandated monitoring inspection was initiated on 08/6/2024 as a part of a conditional license. The on-site inspection began at 11 AM and ended at 1:25 PM. The inspector reviewed compliance in the areas listed above. There were 12 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, observations

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Based on review of five staff's records, the center failed to ensure that no person shall be hired for compensated employment prior to the enter obtaining an employment eligibility letter upon completion of a fingerprint background check as required by the Code of Virginia.

Evidence:

1.Staff # 2 had a documented hire date of 05/28/2024 and on the fingerprint letter of eligibility was dated 06/12/2024.
Plan of Correction: In the future all staff will not start employment until the finger print letter has been returned.
Standard 8VAC20-780-450-C
Based on interviews, the center failed to ensure that all crib sheets were washed daily.

Evidence:

Confirmed by infant room staff the crib sheets were washed weekly.
Plan of Correction: The cribs sheets will be washed daily starting today.
Standard 8VAC20-780-500-B
Based on observation, the center failed to ensure that disposable diapers were disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence:

The storage system for soiled diapers had a swinging lid that the soiled diaper or the staff member?s hand would touch when disposing the soiled diaper.
Plan of Correction: The swinging lid was removed.
