Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 05/22/2025 and completed on 05/28/2025. The on-site inspection began at 10:06 AM and ended at 11:30 AM. The inspector reviewed compliance in the areas listed above. There were 13 children present and 4 staff. The inspector reviewed 5 children?s records, 3 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center shall maintain and keep at the center a separate record for each child enrolled that contains all the required information.

The record for Child #1, enrolled for three months, was missing the following required components: Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: The director will have the parents complete the emergency contacts with someone other than themselves.
