A subsidy health and safety initial inspection was conducted on April 15, 2024 from 3:00 P.M.-3:45 P.M. The Inspector reviewed the provider records submitted by the facility to ensure documentation was complete. The physical plant and emergency plan was inspected. Subsidy requirements were reviewed.
