A subsidy health and safety inspection (SHSI) was initiated and completed on March 19, 2025. The inspection started at 3:21 p.m. and concluded at 4:00 p.m. There was 1 child present and 1 caregiver; the point total was 2. The inspector reviewed 2 children?s records and 1 caregiver record on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-190-B
The child's information shall include all the requirements for their file. The records of children #1 and #2, who had been in care for 10 months, were missing the first date of attendance, name, address, and phone number of at least one person to contact in an emergency, information on allergies and intolerances, and actions to be taken in an emergency situation; information on other physical problems; pertinent developmental information and any special accommodations, names of persons other than the custodial parent who are authorized to pick up the child, immunizations, and permission to transport child.

Standard 8VAC20-790-270-A
Poisonous substances such as cleaning agents, disinfectants, and deodorizers, will be stored in areas inaccessible to children. Lysol spray, Kaboom bathroom cleaner, and two cans of air freshener were stored in areas accessible to children in the bathroom and kitchen.

Shelter-in-place procedures shall be practiced twice a year. The vendor stated shelter-in-place drills have not been practiced.

The vendor shall maintain a record of the dates of the practice drills for one year. For vendors offering multiple shifts, the simulated drills shall be divided evenly among the various shifts. There was no documentation of drills being practiced in 2024.
