Inspector Notes
An unannounced, on-site focused monitoring inspection was initiated and completed on 12/27/2024, as a part of the conditional licensure period. The on-site inspection began at 9:00am and ended at 10:30am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 2 children present and 2 caregivers; the point total was 6 points. The inspector reviewed 2 children?s records and 4 caregiver and household member records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. There were no physicals in the record for Child #1 and Child #2. The children had been attending for more than 50 days.
Plan of Correction: The provider will contact the parent to request that physical examination documentation be submitted.
Standard 8VAC20-800-200-C
Documentation of the orientation shall be signed and dated by the provider and substitute provider or by the provider and assistant. The record for Caregiver #2 did not contain documentation of orientation training.
Plan of Correction: The provider will update caregiver records to include orientation training documentation.
Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe and operable condition. Unsafe conditions shall include, but not be limited to, the presence of splintered, cracked, or otherwise deteriorating wood. There was a broken picket on the fence in the backyard.
Plan of Correction: The broken picket will be repaired or replaced.

Documentation shall be maintained of emergency evacuation drills that includes all of the required information. The documentation for emergency evacuation drills did not include the weather conditions at the time of the drill.
Plan of Correction: The provider has added the weather condition category to the drill log.
Standard 8VAC20-800-840-B
Information recorded for an injury requiring first aid shall include the date and time of the injury and the date and time the parent was notified of the injury. Two injury reports in November did not include the time the injury occurred and the time the parent was notified.
Plan of Correction: The provider will document the time an injury occurred and the time the parent was notified.
