Inspector Notes
An announced on-site initial inspection was conducted on 5/8/24. The inspection began at 2pm and concluded at 3:20 pm. The Inspector reviewed compliance in the areas listed above. There were two- 3 year old children present with the provider for a total of 4 points. There was ample supplies, furnishings and space for the requested capacity and age range.The children were transitioning to naptime but ended up engaged in a painting activity and free play. Four children's records were reviewed on-site. This inspection included document review, a tour of the entire home, interviews and observations. The basement will be the primary daycare location. Information gathered during the inspection determined non-compliance with applicable standards or laws, and violations are documented on the violation notice issued to the program.
Violations
4Child A's file did not contain a second emergency contact or insurance policy number.
Child B's file did not contain an address for an emergency contact, physician address and phone number nor insurance policy number. Child A enrolled on 11/27/23 and child B enrolled on12/4/23.
The parents of child A,B,C and D were not provided in writing information on the provider's medication policy. Child C enrolled on 12/1/23 and child D enrolled on 11/7/23.
911, the number to poison control and the emergency backup person's number were not posted near the phone.
There was no written snack menu.
