Inspector Notes
An unannounced, on-site monitoring and 60-day IPOC inspection was initiated on 10/21/2025 and completed on 10/21/2025. The on-site inspection began at 9:10 am and ended at 12:55 pm. The inspector reviewed compliance in the areas listed above. There were 53 children present and ten staff. The inspector reviewed five children?s records and five staff records on-site on 10/21/2025.
This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/5/2025. A POC submitted after this date will not appear on the public website.

4Staff #4's updated screening was three months overdue. The updated screening has not yet been completed and staff was observed working with children.

On the preschool playground, there were two types of playing equipment with rust and chipping paint: the metal green riding equipment and a red metal house.
On the toddler playground, a colorful push-car had several broken areas, presenting sharp ridges.
On the school-aged playground, the mulch in the fall zones of the slides measured 3 inches. The required resilient surfacing is six inches.
