Complaint Related
Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent, and meet additional qualifications.

Staff reported during interviews on February 5, 2026, that assistant teachers are expected to operate and supervise classrooms without a lead qualified teacher, when the lead teachers are not present or absent. Staff stated that a staff member under the age of 18 years old, was left unsupervised, alone with children for approximately four hours.
Plan of Correction: Identified Leads in classrooms and education qualifications. Ensured all staff members under age 18 remain with qualified staff.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff #1 completed the preservice training 96 days after first date of employment.
Plan of Correction: All staff have been notified to complete Pre-Service training (10 hours) and the 2026 (3hr.) update.

The following staff records shall be kept for each staff person: Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.

Staff reported during interviews that Staff #1 and Staff #2, are used as lead teachers, when needed. Staff #1 and Staff #2, did not have the educational documentation, required for their position, available in the files for review.
Plan of Correction: Site is under new management and all staff files are being audited to ensure files are up to date and all required VA licensing and VDOE paperwork and trainings are completed. Estimated completion 4/11/26.

All staff who work directly with children shall have within 90 days of the date of
employment a current certification in cardiopulmonary resuscitation (CPR) appropriate
to the ages of children in care.

Staff #3 and Staff #4, did not have a current certification in cardiopulmonary resuscitation (CPR). Staff #3 is overdue by four months and Staff #4 is overdue by three weeks.
Plan of Correction: CPR scheduled for 3/19/2026 at 6:30 pm. All staff who needed to renew or certify has been registered.
