During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on 4/8/2025 and completed on 4/8/2025. The inspection started at 10:30 am and concluded at 11:25 am. There were 3 children present, 1 vendor, and 1 household member; the point total was 8. The inspector reviewed 4 children?s records and 2 vendor/household member(s) records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/15/2025.
Violations
2The records for Child #1 and Child #4 were missing the employment information and the employment phone number for the two parents.
The records for Child #1, Child #3, and Child #4 were missing the home address for each parent who has custody.
The records for Child #1 and Child #2 were missing written authorization for emergency medical care should an emergency occur.
The record for Child #1 is missing the address of at least one person designated by the parent to contact in case of an emergency, if the parent cannot be reached
The record for the vendor did not contain documentation of the annual 3-hour updated subsidy training course for 2024.
