During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 11/06/2024, as a part of the conditional licensure period. The onsite inspection began at 9:15am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. At the beginning of the inspection, there were 5 children present and 1 caregiver; the point total was 13. The inspector reviewed 4 children?s records and 2 caregiver records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-120-B
Providers must maintain a record for each caregiver that contains all required information. The record for Caregiver #2 did not contain documentation of verification of age. The Caregiver was alone with with children and has been employed for 3 weeks.
Plan of Correction: The provider will make a copy of the caregiver's form of identification for the record.
Standard 8VAC20-800-150-D-1
Assistants who are left alone with children in care are required to have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. Caregiver #2 was alone with children and did not have current certification in CPR.
Plan of Correction: Provider is current in CPR and first aid. Provider will make sure any caregiver left alone with children is certified in CPR and first aid.
Standard 8VAC20-800-150-D-2
Assistants who are left alone with children are required to have current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. Caregiver #2 was left alone with children and did not have a current certification in first aid.
Plan of Correction: Provider is current in CPR and first aid. Provider will make sure any caregiver left alone with children is certified in CPR and first aid.
Standard 8VAC20-800-540-A
Feeding or activity tables shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened. Two children were observed sitting in a feeding/activity table and were not securely fastened with the safety strap. One child climbed out of the seat independently.
Plan of Correction: The activity table will not be used for children able to independently remove themselves from the table.
Standard 8VAC20-800-60-A
An up to date record for each child enrolled shall be maintained by the provider. There was no record for child #4, who was in care during the inspection and had been in care for one day.
Plan of Correction: The provider printed the record from her phone once she arrived back at the family day home.
Standard 8VAC20-800-60-B
Providers are required to keep a separate record for each child enrolled that contains all of the required information. The record for Child #1 was missing the policy number of the child's medical insurance. The child was present during the inspection and had been in care for one month.
Plan of Correction: Provider will request the information from the parent.
Standard 8VAC20-800-680-B
Caregivers are required to ensure children's hands are washed with liquid soap and warm running water when their hands are dirty. A child was eating with their hands, leaving residue of the food they were eating on their hands. The caregiver did not wash the child's hands after they finished eating.
Plan of Correction: Children's hands will be washed when required.
Standard 8VAC20-800-90-A
"Before attendance, documentation of immunizations is to be obtained for each child.
The documentation of immunizations for Child #2 was dated 2 weeks after the child's first day. The documentation of immunizations for Child #5 was dated 3 weeks after the child's first day.
Plan of Correction: Immunization records were obtained and are in the children's files. Immunizations will be obtained prior to a child's first day.
