Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on October 17, 2024, as a part of the conditional licensure period. The on-site inspection began at 10:00amand ended at 11:00am. The inspector reviewed compliance in the areas listed above. There was one child present and one caregiver; the point total was 4. The inspector reviewed four children?s records and six caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Child #3, who has been in care for over 30 days, did not have documentation of a physical examination in the record.
Plan of Correction: The provider will request a copy of the physical examination of Child #3 and document the record.
Standard 8VAC20-800-60-B
Children's records are required to have the policy number of the child's medical insurance.

The records for Child #1, #3, and #4 did not have the policy numbers of the medical insurances.
Plan of Correction: The provider will ask the parents of Child #1, 3, and #4 to document the children's records with the medical insurance policy number.
