Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on October 9, 2025, as a part of the licensure period. The on-site inspection began at 10:45am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were three children present and two caregivers; the point total was 9. The inspector reviewed four children?s records and six caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.

Child #2, #3, and #4 did not have documentation of additional immunizations once every six months. Child #2, 22 months, last documented immunization was 10 months ago. Child #3, 15 months, last documented immunization was 10 months ago. Child #4, 15 months, last documented immunization was 10 months ago.
Plan of Correction: The provider will contact the parents of Child #2, Child #3, and Child #4 to obtain additional immunization records for each child and document the record. Moving forward, the provider will ensure to obtain documentation of additional immunizations for children under the age of two years old.
