During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on May 21, 2025 and completed on May 21, 2025. on-site inspection began at 1:55 PM and ended at 3:15 PM. The inspector reviewed compliance in the areas listed above. There were 12 children present and three caregivers; the point total was 23. The inspector reviewed five children?s records and three caregiver records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-200-C
Documentation of the orientation shall be signed and dated by the provider and substitute provider or by the provider and assistant.

1. Caregiver #2, who has been employed for 16 days, did not have documentation of orientation on file.
2. Caregiver #3, who has been employed for 336 days, did not have documentation of orientation on file.

Poisonous materials shall be stored in an area inaccessible to children.

1. A container of Febreze and a container of Clorox were on a stand in the hallway within reach of children.
2. A container of Clorox was on a shelf within reach of children in the room where children have meals.

Cribs, cots, rest mats, and beds when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges.
