Inspector Notes

An in-person initial subsidy inspection was conducted on 4/01/2024 from 9:55am to 10:40am. There were no children in care with the vendor. The inspector reviewed (4) adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Based on review, the vendor did not have the fingerprint results or the sworn disclosure statement for one household member.
Evidence:
1. Adult Household member #2 did not have the fingerprints completed and did not have the sworn disclosure statement on file with the County.
Plan of Correction: We will get it scheduled this week and will have them sign the Sworn Statement.

Based on observation, the first aid kit did not contain the minimum requirements.
Evidence:
1. The first aid kit was missing the tweezers.
Plan of Correction: I will buy one today.
