During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 1/24/2025. The inspection started at 1:30pm and concluded at 2:30pm. There was one child present and one caregiver; the point total was three. The inspector reviewed two children?s records and four caregiver/household members records on-site on 1/24/2025..
This inspection included document review, tour of the home, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/03/2025.

Standard 8VAC20-790-190-B
Each child's record should include all required information.

Child #1's file was missing each custodial parent's work phone number and proof of the child's identity,
Child #2's file was missing each custodial parent's work phone number, proof of the child's identity, written authorization for emergency medical care should an emergency occur
and the parent cannot be located immediately unless the parent presents a written
objection for the provision of medical treatment on religious or other grounds and a written agreement between the parent and the vendor.
Plan of Correction: Parent will be asked to provide work phone number and proof of children?s identity, and sign emergency written authorization

Each child's record should include permission to transport child if the vendor provides transportation.

Child #1 and Child #2's file was missing permission to transport child if the vendor provides transportation and the vendor provides transportation.

Child #2 (1 year 8 months old) was observed sleeping in a playpen on top of a pillow and blanket.
Plan of Correction: All children who fall asleep in play space other than own rib, cot, mat, or bed will be moved to their designated sleeping space

If play yards, portable cribs, or mesh-sided cribs are used for sleeping or napping, they shall meet the requirements of subsections H through L of this section.

Child #2 (1 year 8 months old) was sleeping in a play pen and the vendor did not have record that it meets the requirements of subsections H through L of this section.
Plan of Correction: Play yards, portable cribs, or mesh-sided cribs will be checked if they meet requirements to sleep or napping.

Cots, rest mats, or beds shall be provided for children 12 months of age or older and shall not be occupied by more than one child at a time.

Child #2 (one year and eight months) was not provided a cot, rest mat or bed to sleep on and was observed in a play pen sleeping.
Plan of Correction: All children will be provided a cot, rest mat, or bed. In this instance, children #2 will only sleep in the crib

The vendor stated the mattress on the floor in the bedroom and the crib in the bedroom were used for children in care to sleep and were not covered with a waterproof material that can be cleaned and sanitized.

The diapering surface shall be separate from the kitchen, food preparation areas, or surfaces used for children's activities, nonabsorbent and washable; and cleaned and sanitized after each use.

The vendor stated she changes the diapers on the crib mattress in the bedroom and washes the sheets everyday. The mattress did not have a plastic liner, is used for sleeping and is not nonabsorbent.
