During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 12/10/2024, as a part of the conditional licensure period. The inspection began at 9:30 AM and ended at 10:50 AM.. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregivers; the point total was 4. The inspector reviewed 2 children?s records and 2 caregiver/household member records. This inspection included:
? document review,
? tour of the facility,
? interviews, and
? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 20, 2024. A POC submitted after this date will not appear on the public website.
Violations
3The provider did not have documentation of obtaining the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from a state in which the provider and two adult household members resided in the preceding five years.
Provider files contains documentation of the results of the background checks from a state in which the provider and two adults of the household members resided in the preceding five years.
The records for child 1 and child 2 do not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of emergency medical treatment on religious or other grounds.
The records for child 1 and child 2 did not contain the addresses of emergency contact persons. The record for child 2 did not include the name, home address, telephone number, and the name address and telephone number of the place of employment for one parent.
Child 1 and child 2 records contain written authorization for emergency medical care in the absence of the parent. Child 1 and child 2 contain addresses of the emergency contact persons.
The records for child 1 and child 2 did not contain the following:
- Whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 22.1-289.050 of the Code of Virginia;
- Requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life- threatening diseases, which must be reported immediately;
- Requirement for the child to be adequately immunized as required by 8VAC20-800-90;
- Requirement for paid caregivers to report suspected child abuse or neglect according to ? 63.2-1509 of the Code of Virginia;
- Custodial parent's right to be admitted to the family day home any time the child is in care as required by ? 22.1-289.054 of the Code of Virginia;
- General daily schedule that is appropriate for the age of the enrolling child;
- Discipline policies including acceptable and unacceptable discipline measures;
- Amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider;
- Provisions of the family day home's emergency preparedness and response plan; and
- Address of the website of the department, with a note that a copy of this chapter and additional information about the family day home may be obtained from the website, including compliance history that includes information after July 1, 2003.
Child 1 and child 2 have been updated to include the listed required standard.
