During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/06/2026, as a part of the licensure period. The on-site inspection began at 10:05 AM and ended at 11:20 AM. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 8. The inspector reviewed 4 children?s records and 3 caregiver/household member records on-site.
This inspection included:

? tour of the facility,
? interviews, and
? observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
The provider's most recent tuberculosis screening was obtained more than two years ago.
The tuberculosis screening for household member 3 was not dated.
Plan of Correction: Provider response:
The provider will obtain a repeat tuberculosis screening.
A tuberculosis screening for household member 3 that includes the date of the screening will be obtained.
Standard 8VAC20-800-60-B
A child's record shall contain all required information.
The record provided for child 2 did not include the parent's work telephone number and no information for persons to be contacted other than the parents in an emergency when a parent cannot be reached.
The record provided for child 3 did not contain the complete street address for a person to be contacted in an emergency if a parent cannot be reached.
