Inspector Notes

An initial subsidy health and safety (SHSI) inspection was conducted on-site August 19, 2024 from 10:11am until 11:51am. There were 14 children present with 6 staff supervising. The physical plant, emergency plan, and subsidy requirements were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 28, 2024. A POC submitted after this date will not appear on the public website.

Based on document review and interview, the vendor failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: The attendance record for classroom 3 documented 9 children in care. 10 children were observed in attendance. Staff #5 confirmed that one child was not recorded on the log.
Plan of Correction: The director has made hard copies for attendance records. Staff started using them as of 8/23/24.
